Logos Information Form
Event Form If you have an upcoming event that needs church facilities or you would like en event placed on the church calendar and /or advertised in the weekly bulletin, please fill out this Calendar Event Form for submission to the church office.
Dedication Baby Dedication form to schedule a time during an upcoming Sunday Morning service to dedicate infants or children to the Lord. Please fill out and submit to the church office.
Driver Information
Equipment Check Out Form must be completed when any equipment is to be used off HNC location. You will accept all responsibility for the equipment being checked out until it is returned.
Van Request Form Printable form used when requesting use of church vans. After completing form, please turn it in to the church office for approval.
Teen Medical Release Form Printable form to be completed by parents of teens participating in summer camps and other church sponsored events. Please turn in to Pastor Chris Canton.
Expense Form Expense re-embursment form for authorised church purchases.
