Baby Dedication form to schedule a time
during an upcoming Sunday Morning service
to dedicate infants or children to the Lord.

Check Out
Form must
be completed when any equipment is to be
used off HNC location. You will accept all
responsibility for the equipment being checked
out until it is returned.
